In past years, the district was required to issue 1095 forms to employees regarding Employer Provided Health Insurance for tax purposes. In December of 2024 Congress passed the Paperwork Burden Reduction Act as well as the Employer Reporting Improvement Act which removed the requirement for employers to send 1095 forms to employees unless requested.*

If you would like a copy of your 1095 form, please contact Nicole Garrett, Human Resource Coordinator by email at [email protected]. You will receive the form within 30 days of your request. You may receive the form electronically with your consent.

If you have any questions regarding this please reach out to Nicole Garrett, [email protected]
